Schedule: Full-Time, Tues – Sat or Sun-Thurs

Salary Range: $110,000.00 To $120,000.00 Annually

The Wellness Nurse is responsible for the coordination, supervision, and oversight of clinical services within assigned areas of the community. This role supports the Director of Nursing by ensuring nursing operations, documentation, medication management, and regulatory compliance are maintained in accordance with federal, state, and local requirements.


Essential Duties and Responsibilities

Clinical & Administrative Oversight

  • Supervise nursing staff, caregivers, and medication assistants

  • Provide operational coverage in the absence of the Director of Nursing

  • Monitor staffing assignments and ensure clinical coverage is maintained

  • Participate in interdisciplinary meetings as required

Resident Care & Documentation

  • Assist with pre-admission evaluations and Individualized Service Plans (ISPs)

  • Conduct assessments and reassessments and document changes in condition

  • Ensure timely and accurate clinical documentation in resident records

  • Coordinate care with physicians, pharmacy, hospice, rehab, and home health agencies

Medication Management

  • Oversee medication ordering, storage, administration, and documentation

  • Ensure compliance with medication administration standards and protocols

Quality Assurance & Compliance

  • Participate in Quality Assurance meetings and audits

  • Ensure adherence to clinical policies, procedures, and regulatory standards

  • Maintain resident confidentiality and HIPAA compliance

Communication

  • Communicate with families, providers, and internal departments as required

  • Support Neighborhood Managers with resident care coordination and operational needs

Emergency & Operational Support

  • Assist with emergency preparedness activities and drills

  • Support operational needs of the neighborhood as assigned


Qualifications

  • Licensed Registered Nurse (RN) in the State of New York

  • Bachelor’s degree preferred

  • Minimum of 3 years of supervisory or leadership experience in senior living or healthcare

  • Ability to work flexible schedules, including evenings and weekends

  • Proficient in electronic health records and Microsoft Office


Physical & Mental Requirements

  • Ability to stand, walk, sit, and perform repetitive hand movements

  • Ability to lift up to 25 pounds and assist with resident mobility as needed

  • Ability to remain focused and professional in a fast-paced clinical environment
